Position:  Staff CNA (2 Positions Available)

  • Shift Hours:  PRN 7A-7P and/or 7P-7A
  • The candidate selected for this position will provide care for patients on the Medical Floor as assigned by the Charge Nurse. 
  • Must be able to work well under stress and prioritize.  Must attend staff meetings regularly and comply with policies or protocols.  Current CPR certification will be required.  Must be flexible and work well as a team member.  Must be self-motivated and function independently.  Must be able to set goals and meet deadlines efficiently and effectively.

Position:  Staff RN (2 Positions Available)

  • Shift Hours:  Full Time 36 hours per week 7A-7P with every other weekend and holiday rotation.
  • The candidate selected for this position will provide care for patients on the Medical Floor, Emergency Department and Specialty Clinics as assigned.  The RNs selected will also be trained to fill in when necessary as Charge Nurses.
  • Must be able to work well under stress and prioritize.  Must be self-motivated and lead by example.  Must attend staff meetings regularly and assist to implement any new policies or protocols.  ACLS and PALS will be required. Must be flexible and work well as a team member.  Must be self-motivated and function independently.  Must be able to set goals and meet deadlines efficiently and effectively.  Recovery room experience with ENT and Pediatrics preferred.

Position:  Specialty Clinic LPN-PRN
(2 Positions Available)

  • Maintains an environment of safety for patients, self and others and supports patient safety initiatives.
  • Prepares patients and assists physician in assessment and examinations of patient including assessment of patient safety needs.
  • Assists physician in performance of diagnostic procedures, treatments, administers medications, and performs related tasks as directed.
  • Documents entries into medical record according to established policies and procedures.
  • Initiates communications to patients according to individualized needs of the patient as prescribed by the physician, including instructions, planning, and triaging with full consideration of patient safety needs.
  • Performs disinfecting procedures.
  • Cleans and maintains examination rooms and other assigned areas.
  • Documents safety and effectiveness of patient care equipment into logs.
  • Participates in performance improvement and quality improvement activities.
  • Recognizes, supports, and utilizes evidence based nursing practices.

Position: Utilization Review/Discharge Planner/Swing Bed Coordinator
(1 Full Time Temporary and 1 PRN Position Available)

  • Shift Hours: Full Time 40 hours per week. Work hours will primarily fall between Monday - Friday. On call coverage will be required every other weekend. Adjustment of work hours may be required for duties that occasionally fall outside the regularly schedule shift.
  • The position will require reviewing all observation and inpatient charts for medical necessity and gathering information from the chart to perform concurrent review using Milliman Care Guidelines (MCG). Information is reported as required to Medicaid, Managed Care Organizations (MCOs) or commercial insurances as needed to obtain authorization for patient stays or services.
  • The person selected for this position must be available to counsel both acute and long term care patients regarding social needs and have an understanding of the social and emotional needs of the patient and family. Discharge planning activities including discussions with patients and families as indicated for long term care placement, home health follow up, community services and other discharge related services. Coordinate, document and assure compliance with Conditions of Participation (CoPs) defined by the Centers for Medicare and Medicaid Services (CMS) for Swing Bed stays. Marketing and education regarding the Swing Bed program with area hospitals, facilities and providers is required. Marketing and education may include phone contact, distribution of information, face to face meetings, or any other activity necessary to promote the Swing Bed program.
  • Must be able to prioritize and work well under stress. Must be flexible, work as a team member, be self-motivated and function independently. Must be able to set goals and meet deadlines efficiently and effectively.
  • Prior experience in Utilization Review, Discharge Planning and Swing Bed Coordination is preferred.
